Why MCS Advisory for ASC Development

Building an ASC is complex. We simplify the journey.

From strategic planning to opening day, ASC projects involve critical decisions, regulated timelines, and an endless list of tasks. With over 20 years of experience in ASC development and operations, MCS Advisory has successfully guided projects ranging from single-room facilities to multi-room, multi-specialty state-of-the-art centers. We have partnered with solo practices, multi-specialty groups, and single-specialty physician-owned facilities across the country.

No two ASCs are alike. MCS customizes every project to align with your goals, specialty mix, and local market conditions — including Certificate of Need (CON) considerations, payor market dynamics, and competing site analysis.

The MCS Methodology

ASC development process — step by step.

A proven, sequenced framework refined across twelve successful turnkey ASC projects. We adapt the cadence to your CON status, specialty mix, and physician-investor structure.

Phase A

Project Introduction & Feasibility Assessment

  • Meet with core physicians to define goals and vision for the ASC
  • Conduct site selection to identify an optimal location
  • Analyze market dynamics — competition, payor mix, and Certificate of Need (CON) requirements
  • Execute a Non-Disclosure Agreement (NDA) with prospective partners
  • Gather case and procedure volumes with associated CPT codes
  • Develop an initial financial proforma for feasibility
  • Review potential site with an architect and create an initial schematic
  • Hold a second meeting with additional physicians to refine case volume projections
  • Revise the proforma based on physician feedback and updated case volumes
  • Secure signed Letters of Intent (LOIs) from participating physicians
Phase B

ASC Contractual Phase

  • Obtain the Tax ID for the LLC or partnership entity
  • Draft initial partnership and operational agreements (in coordination with healthcare counsel)
  • Interview and select an architect with ASC-specific experience
  • Execute the AIA document for architectural services
Phase C

Plans & Construction Phase

  • Finalize the revised schematic with partnership approval
  • Complete construction plans and documents with the architect
  • Submit plans for approval to state health and local building authorities
  • Define IT and clinical technology requirements (network lines, EMR integration, drops)
  • Manage bidding from qualified general contractors (GCs)
  • Select a GC and execute the construction contract
  • Begin demolition and secure local permits
Licensing & Certification

State licensure. Medicare certification. Accreditation-ready.

Opening an ASC requires meticulous adherence to state and federal regulations. MCS Advisory guides you through every step to ensure compliance and expedite approvals — including state licensure, Medicare certification, AAAHC accreditation, ACHC accreditation, and Joint Commission readiness.

Step 1

State Licensure

To participate in the Federal ASC program and receive facility fees from governmental payers, an ASC must first meet state licensure requirements. State surveys vary by jurisdiction; common requirements include physical plant standards, infection control protocols, staffing minimums, and clinical policy documentation. MCS ensures your facility is survey-ready and meets every requirement before the state inspector arrives.

Step 2

Medicare Certification

After state licensure, the ASC must meet the Centers for Medicare and Medicaid Services (CMS) Conditions for Coverage to achieve Medicare certification. To ensure a smooth certification process, we guide you through:

  • Hiring the ASC Administrator at least 60 days before opening day
  • Hiring core clinical staff four weeks before obtaining the certificate of occupancy
  • Performing eight test cases for Medicare accreditation submission
  • Managing credentialing forms for other insurance providers and commercial payors
  • Coordinating AAAHC, ACHC, or Joint Commission accreditation surveys
What to Avoid

Common ASC development pitfalls.

Twelve turnkey projects later, these are the patterns we routinely see derail otherwise-promising ASC ventures. With MCS, you sidestep them.

Mis-sized facility
Overbuilding (capital trapped in unused capacity) or underbuilding (constrained growth, reconstruction within 5 years).
Prolonged licensure timelines
Submitting state plans that fail review, requiring resubmission and pushing certificate of occupancy beyond physician case-volume commitments.
Governance and operational lag
Delayed administrator hire, unfinalized clinical policies, and missing accreditation paperwork that postpones first revenue.
Insufficient physician engagement
Physician-investors disengaging during construction, leading to case volume falling short of proforma assumptions at go-live.
Partnership

From concept to completion.

MCS Advisory provides a full-service team across finance, healthcare law (via Noel C. Pace, FACHE), and strategic accounting (via Adam) for ASC development. Whether you are an independent physician group, a multi-specialty consortium, or part of a healthcare system, our customized approach ensures a seamless process — from initial feasibility through first Medicare-certified case.

ASC work at MCS spans two of our practice lanes: Transaction Advisory (financial structuring, physician syndication, MSO integration) and Fractional CFO (operational launch, payor contracting, accreditation, revenue cycle).
