Project Introduction & Feasibility Assessment
- Meet with core physicians to define goals and vision for the ASC
- Conduct site selection to identify an optimal location
- Analyze market dynamics — competition, payor mix, and Certificate of Need (CON) requirements
- Execute a Non-Disclosure Agreement (NDA) with prospective partners
- Gather case and procedure volumes with associated CPT codes
- Develop an initial financial proforma for feasibility
- Review potential site with an architect and create an initial schematic
- Hold a second meeting with additional physicians to refine case volume projections
- Revise the proforma based on physician feedback and updated case volumes
- Secure signed Letters of Intent (LOIs) from participating physicians
ASC Contractual Phase
- Obtain the Tax ID for the LLC or partnership entity
- Draft initial partnership and operational agreements (in coordination with healthcare counsel)
- Interview and select an architect with ASC-specific experience
- Execute the AIA document for architectural services
Plans & Construction Phase
- Finalize the revised schematic with partnership approval
- Complete construction plans and documents with the architect
- Submit plans for approval to state health and local building authorities
- Define IT and clinical technology requirements (network lines, EMR integration, drops)
- Manage bidding from qualified general contractors (GCs)
- Select a GC and execute the construction contract
- Begin demolition and secure local permits

State Licensure
To participate in the Federal ASC program and receive facility fees from governmental payers, an ASC must first meet state licensure requirements. State surveys vary by jurisdiction; common requirements include physical plant standards, infection control protocols, staffing minimums, and clinical policy documentation. Medicare Certification
After state licensure, the ASC must meet the Centers for Medicare and Medicaid Services (CMS) Conditions for Coverage to achieve Medicare certification. To ensure a smooth certification process, we guide you through:

- Hiring core clinical staff four weeks before obtaining the certificate of occupancy
- Performing eight test cases for Medicare accreditation submission
